Is a collared shirt required or is a dry fit performance shirt acceptable?

Collars are not required. Dri fits are a great idea.

If i am on the waitlist to play ma50 and i don't get pulled off the waitlist to play by Friday will i get a refund

The waitlist is changing constantly. If we reach a point where you believe you wont get in, even up to the point of the start of the tournament, yes you'll be refunded.
